NEW Patient Appointments
Welcome to our practice !!! In order for us to serve you more efficiently and effectively, we ask that you TEXT our office at 828-893-0010 if you will be a new Valdese , NC location patient or text us at 828-758-0010 if you will be a new Lenoir, NC location patient. You may also call us at either location. Please see our office hours for each location prior to calling or texting. We will need your HIPAA consent in order to send you our New Patient Intake Forms via Link right to your smart device.
Patients of Record
You may text our office 24/7 for any needs or appointment requests that you may have. Please note if this is during a time we are out of the office, on the weekend or after hours, then your request will be answered on the next business day the office is open. If this is an emergency that cannot wait until the office reopens , please seek help by reporting to your nearest Urgent Care facility or Hospital.
TEXT Messaging Disclosure
Please know that text messaging is not secure, so in order for us to discuss your PHI we must have a signed HIPAA consent. If you agree, we can text you a HIPAA consent right to your smart or mobile device. Note text messaging rates do apply if you have limitations on your contracted individual plan through your network.
Patient Forms
Please text the location you would like to be seen at for a New Patient or patient of record appointment. We will set up your appointment and text you our digital forms to fill out prior to your appointment, along with a short COVID Screening form. Please keep in mind hours of operation at each location and someone will respond to you as quickly as possible . Once you have requested and received your digital links, please go ahead and fill out these forms to ensure we are taking the absolute best care of you at your visit.
